Perbedaan Status Oksigenasi pada Posisi Semi Fowler dan Fowler Pasien Pneumonia

Abstract

Oxygenation status is a key indicator in assessing the condition of pneumonia patients whose condition is compromised by inflammation and secretion buildup. An effective strategy to improve oxygenation status is to reposition the patient into the semi-Fowler or Fowler position. This study aims to analyze differences in oxygenation status among pneumonia patients placed in the semi-Fowler and Fowler positions.

This study employed a quasi-experimental design with a crossover approach involving 54 pneumonia patients selected using consecutive sampling. Data were collected using an oxygenation status observation sheet, including SpO₂, respiratory rate, dyspnea severity, and use of accessory breathing muscles. Each indicator was scored on a scale of 1 to 3 based on severity. Subsequently, patients were placed in the semi-Fowler and Fowler positions for 20 minutes. Oxygenation status was measured before and after the intervention, with a 20-minute rest period between the two interventions. Data were analyzed using the Wilcoxon test.

Test results showed a p-value of 0.029 (p<0.05), indicating a significant difference in oxygenation status between the semi-Fowler and Fowler positions, with an average score of 9.56 for the semi-Fowler position and 9.19 for the Fowler position. The results also indicated that oxygenation status in patients in the semi-Fowler’s position was better than in those in the Fowler’s position.

Placing patients in the semi-Fowler’s position can help reduce diaphragmatic pressure, increase comfort, and improve breathing patterns, thereby effectively enhancing oxygenation. Therefore, the semi-Fowler’s position is recommended for patients experiencing shortness of breath, particularly those with pneumonia, to help improve their oxygenation status.
