ASUHAN KEBIDANAN BERKELANJUTAN (CONTINUITY OF CARE) PADA MASA PRAKONSEPSI DI WILAYAH KERJA PUSKESMAS ANDALAS KOTA PADANG TAHUN 2025
Abstrak
Objective: Maternal mortality remains a major public health problem, and many risks can be prevented through early identification before pregnancy. Preconception care is essential to improve women’s readiness for pregnancy. This study aimed to describe the implementation of Continuity of Care (CoC) during the preconception period in the working area of Andalas Public Health Center, Padang.
Methods: This study used a descriptive case study design. Four women in the preconception period received comprehensive midwifery care based on assessment, diagnosis, planning, implementation, and evaluation. Data were collected through interviews, physical examinations, observations, and documentation review, then analyzed descriptively.
Results: The implementation of CoC helped identify the women’s health problems and knowledge needs. Continuous care improved understanding of preconception health, including nutrition, personal hygiene, immunization, and preparation for pregnancy, and supported positive changes in health-related behaviors.
Conclusion: CoC during the preconception period enhances women’s readiness for pregnancy by improving health knowledge and behaviors. Strengthening preconception services as part of comprehensive midwifery care is recommended.
