PLENARY SESSION: IMPLEMENTATION OF EBP IN CLINICAL PRACTICE: THREAT AND CHALLENGE

Abstrak

Evidence-based practice (EBP), which integrates the best research evidence with expert opinion as well aspatient values to solve clinical problems, has been considered as aneffective strategy for improving the quality of care. To employ EBP has been addressed as one core competence of all clinicians. However, two crucial issues are worth for attention. First, the economic, social policy, and scientific factors are general reasons for failure of research translation to practice. The mismatch between an intervention and the reality leads to low adoption and implementation. Second, overcoming barriers of implementing EBP is organizational challenge. Common barriers of implementing EBP include lack of knowledge, insufficient resource, lower motivation resulted from workload, less interest in reading research articles, and language barrier in non-English countries.

Key elementsof an organization could initiate are (a) leadership’s support for an EBP culture, (b) enlarge the capacity to engage in EBP, allowing nurses to think beyond routines and inspirit them to search for the best care to meet patients’ needs, and (c) the infrastructure to support and maintain an EBP culture and related activities.The organization could apply different strategies based on their resources.As an example in the Nursing Department of University Hospital of NCKU, Taiwan where initiating implementing EBP from 2004. We organized an EBP taskforce group and annual plan was made to achieve the goal of the department and hospital. Annual training program was held to cultivate EBP seed. We also incorporated competence evaluation of EBP to the nurses’ clinical ladder promotion system. Competences vary with different ladders and continuous education. Developing or modifying clinical practice guideline and collaborating with other disciplinary were required for advanced practice nurses. In addition, we organized journal reading club to help EBP seeds to continuously develop their competence in teaching. Through the strategies planning, we created a sustainable system in conjunction with policy and cultivate an EBP culture.
